Celebrate With Us
Birthday Club Registration:
To participate in the Milk & Cookies Foundation Birthday Club, all individuals must complete registration at least 60 days before their birthday month. Early registration allows our team enough time to properly plan and celebrate each individual during their special month.
Participants will also have the opportunity to vote on monthly birthday themes, helping create fun, memorable, and inclusive celebrations for everyone involved.
All registrations must be submitted to:

• Parent/Guardian/Caregiver Name and Contact Information
Birthday Token
At Milk & Cookies Foundation, we want to help make every birthday special. Each individual celebrating their birthday with us will receive a Milk & Cookies Foundation T-shirt along with a special birthday token as part of their celebration.
Please include T-shirt size during registration to ensure each participant receives the correct size.

The Milk & Cookies Foundation Birthday Club offers separate celebrations to ensure activities and themes are age-appropriate for all participants.
Children & Teen Celebrations: Ages 5–17
All participants under 18 must have a parent or guardian complete registration and provide contact information during enrollment.

Supervision Requirement
Participants ages 18 and older who require supervision or personal assistance must have a parent, guardian, or caregiver remain with them during the event to help ensure their safety, comfort, and support needs are properly met.
